Care traffic
Look closer: the word is the network. Every letter is a route between care organizations, and the dots are referrals and handoffs moving along it. Illustration.

Carely Eco

Coming soonCarely Eco is under build.

The coordination infrastructure for connected care

Free profiles. Building-block setup. Referrals, handoffs and shared accountability between the organizations that already work together.

Built for every organization along the journey

The platform

One shared layer between the systems you already use.

Carely sits between organizations, not inside them. Three building blocks make every handoff visible, owned and confirmed.

Profiles

Every organization gets a home on Carely: the services it offers, who it serves and what it needs from partners, so the right handoffs find it.

  • Services, capacity and service area
  • Intake requirements and named contacts
  • Who you already work with

Pipelines

Your real workflows, mapped with your team and turned into building-block pathways that connect your steps to your partners’ steps.

  • Start from a ready-made workflow
  • Every step has an owner and a deadline
  • Works alongside the systems you already use

Trails

A timestamped record of every handoff: what is needed, who owns it, what was shared, and when it was accepted and done.

  • Every action names who did it, and when
  • Sealed when the handoff is complete
  • Corrections are added, never overwritten

How it works

How a handoff moves.

Both teams see the same handoff, the same status and the same clock, from the moment it’s sent until it’s sealed.

  1. 1

    Send

Pick a workflow and a partner, fill in one form and send. The receiving team sees it in their inbox right away.

  2. 2

    Accept

They accept in one tap, or decline with a reason you can see. From then on, both teams watch the same status.

  3. 3

    Advance

Each step has an owner and a deadline both teams can see. Either side can add a note or send a nudge.

  4. 4

    Seal

When the last step is done, the handoff is sealed. Its record can’t be edited or deleted.

The handoff

A handoff should be more than a referral.

Every handoff on Carely carries six things, so the next team never has to chase them.

  1. NeedWhat is required?
  2. OwnerWho is responsible for the next action?
  3. InformationWhat does the receiving party need?
  4. StatusWhere is the handoff?
  5. ConfirmationHas the receiving party accepted it?
  6. Next actionWhat needs to happen now?

Why it matters

The cost shows up between the handoffs.

17.4%

of Ontario hospital days in 2023–24 went to patients designated alternate level of care (ALC), waiting in hospital for a more appropriate care setting. That is 1.5 million hospital days in one year: the same as 4,096 beds full all year.

How organizations start

Start with one transition.

The first workflow is usually hospital discharge to the next care environment, mapped and measured with one partner organization.

  1. 1

    Discovery

A one-hour conversation about how one transition works today.

  2. 2

    Map

We map the steps, the people and the gaps with your team.

  3. 3

    Digitize

The agreed workflow becomes a Carely pipeline.

  4. 4

    Pilot

Run it for 30 to 60 days with real users, then measure.

Start with one transition. Build the network from there.
